Systematic Review of Patient Outcomes and Treatment Moderators following Wrist Fractures

Abstract
Introduction: Wrist fractures constitute the most frequently occurring upper limb fractures. The aim of this systematic review is to summarise evidence for the effects of rehabilitation treatment of wrist fractures on pain and functional outcomes and identify potential moderators of these effects.
Method: A search of MEDLINE, EMBASE, CINAHL, AMED, Web of Science, Cochrane Controlled Trials Register, and PEDRO was performed for wrist fractures and associated terms. Eligible studies were randomised controlled trials of adults>45 years that sustained wrist fracture and received non-pharmacological or non-surgical treatment.Results:2876 titles were screened and 12 studies met eligibility criteria. 9/12studies included physiotherapy (PT) and/or occupational therapy (OT) interventions. The remaining 3 studies evaluated isometric massage, compression gloves and whirlpool therapy. Significant effects on reduction of pain and/orgain of functional outcome were found for PT, OT, cross training, compression glove use, and isometric massage. One study reported results of subgroup analysis identifying age as potential treatment moderator, with smaller effects found in older age. Although planned, meta-analysis was not possible.
Conclusion: There was evidence for OT and PT as rehabilitation interventions for pain and/or functional outcome. We found limited evidence only forage as a treatment response moderator.
